We ask that you bring your insurance card to your appointment, a copy will be placed in your file at the time of registration. Our insurance coordinator is familiar with most insurance companies and will file the claim for you, however, remember each plan’s coverage is different. Please understand that you are ultimately responsible for payment of any uncovered portion and all services that are not covered by your insurance.
As one of our payment options, the CareCredit healthcare credit card gives you the flexibility and convenience of managing your out-of-pocket healthcare expenses. With their deferred-interest and low-interest monthly payment plans, you can begin your treatment today. The application process is simple and can be done in our office or at home on your computer.
Rescheduling or Canceling Your Appointment
When you schedule an appointment at our office, we reserve that time and prepare in anticipation of serving you. If you should need to reschedule, we kindly request that you notify us at least two business days prior so that we may have ample time to contact other patients who are awaiting treatment.
We understand that conflicts can arise unexpectedly; however, failing your appointment or canceling without adequate notice more than once may result in a charge.
